Description
  •  Prepare, Assemble And Maintain Patient Records
  •  The Contractor shall assemble initial patient treatment record, utilizing Composite Health Care (CHCS) and Automated Data System (ADS).
  •  Approximately 128,400 TASK 1 transactions will require processing during the period of performance at an average of thirty (30) Task 1 transactions per hour.
  •  Review outpatient-visit documentation and entering of the appropriate ICD-9, CPT, HCPCS and E&M codes.
  •  Adhere to accepted coding practices as prescribed by the Department of Defense, JCAHO, AHA Coding Clinic and CMS coding guidelines.
  •  Enter codes into the Ambulatory Data System (ADS) directly.
  •  Confirm correct patient data is contained in the medical record. Make corrections when required in ADS before entry of the diagnosis and procedure codes.
  •  File all medical records prior to the end of the workday.
  •  Maintain daily job status reports including completed activities, all metrics, incomplete activities and the status of any special interest data projects.
  •  May participate in the development of ad hoc programs using a data retrieval system when required.
  •  Compile, review, and file medical records of hospital or clinic patients collecting statistics for use in reports and surveys.
  •  Print and forward patient records to appropriate admissions.
  •  Review records, assemble forms, eliminate duplication and test results confirming completeness.
  •  Review medical records and associated files annually, pull medical records and X-rays that have not been used for two (2) years.
Qualifications 
  • Contractor personnel shall possess the necessary clerical knowledge and skills necessary to carry out all assigned clerical duties.
  • Personnel shall have a working knowledge of the Microsoft Office Software suite.
  • Must be able to type 40 words per minute possess excellent customer service skills.                                                                       
  • SECURITY REQUIREMENT: Must be able to obtain/maintain a Secret clearance.
  • Please note that pursuant to a government contract, this position requires U.S. Citizenship status.
